Remote Support

Remote support for home healthcare services is a technologically-driven approach aimed at providing assistance and care to individuals in the comfort of their own homes. It enables healthcare professionals to remotely monitor and manage the health conditions of patients without the need for physical presence. It enhances accessibility, convenience, and quality of care while promoting patient empowerment and reducing healthcare burdens.

Daily and Residential Habilitation

Residential habilitation services are programs and supports designed to assist individuals with disabilities in living and participating in their communities. These services aim to enhance their independence and overall quality of life by providing various types of support, such as assistance with daily living skills, personal care, social and recreational activities, and community integration. The goal of residential habilitation services is to empower individuals with disabilities to live as autonomously as possible while ensuring their health, safety, and well-being.

Old Friends

Structured Family Care

Structured Family Caregiving operates by either the caregiver residing in the home of an elderly or disabled individual, or vice versa, where the client moves into the caregiver's residence. The care provider receives financial compensation for delivering care to the elderly person who would otherwise require nursing-home-level care, along with ongoing educational assistance from a care team.

Participant Assistance and Care

Participant Assistance and Care (PAC) Services aim to support individuals with intellectual/developmental disabilities by enabling them to maintain a successful and independent lifestyle in their own homes and actively participate in their communities. These services provide hands-on assistance, prompting, reminders, supervision, and monitoring to ensure the participant's health, safety, and overall well-being. PAC services cover a wide range of areas, including activities of daily living, self-care, and mobility.

Some examples of personal care services include assistance with:

  • Bathing

  • Toileting

  • Dressing

  • Grooming

  • Eating and meal prep

  • Medication reminders

  • Mobility
